Circuit Breaker Application

Circuit Breaker Applications can now be filed on line. Before you begin filling out the application, be sure to have the following information ready:
• Social Security Number, Date of Birth
• Medicare Card and any other drug prescription cards you may have
• Your 2009 W-2 and any indication of income received in 2009
• Any income you have received in 2009
• Your current (liquid) asset balance
• Know the amount of real estate taxes or rent you paid in 2009

Check Your Circuit Breaker Application Status

After you fill out your application on line please allow at least three to four weeks for the processing of your application. Then feel free to click on the above link to see if your application has been processed. Circuit breaker will notify you of any changes that need to be made.
